First Dental Visit

If you are coming to our office for the first time for a routine visit, we'll schedule you for a one-hour appointment during which you will have your x-rays, cleaning, and examination completed.  If you're coming to us for the first time for something else, your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, treatment can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a follow-up appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your first visit:

• A photo ID, insurance card (if applicable) and any x-rays (again, if applicable).

*There are four forms.  Please provide the requested information.  There is no need to print the forms.  We will ask for your electronic signature when you come into the office for your in-person visit.  Also, when completing the forms, please use a Windows-based system and either Internet Explorer or Mozilla Firefox for best results. There are two one-page forms: the Appointment Policy agreement and the Consent for Internet Communications. There are also two longer forms that request your Patient Information and Medical & Dental History. It is important that you fill out these forms in order to provide information that is necessary to best serve your dental needs safely and effectively.

IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.

X-Rays

If your previous dentist has taken recent x-rays (within 6-months), you may request that they forward them to our office. If they are traditional film x-rays and there is not enough time, please pick them up and bring them to our office. If additional x-rays are necessary, digital x-rays can be taken at our facility.
